Eligibility Criteria

• Adults aged 75 and over
• Care home residents (older adult care homes)
• Immunocompromised individuals aged 6 months and over, including:
• Organ, bone marrow, or stem cell transplant patients
• Patients undergoing chemotherapy or radiotherapy
• Those with a history of certain cancers (e.g. chronic leukaemia, lymphoma)
• Individuals with genetic immune disorders
• People on long-term systemic steroids or immunosuppressive therapy
• People living with HIV

Housebound Patients

Patients coded as housebound by their GP will be contacted directly by the Park Practice.
